Our Advanced Illness Management (AIM) Clinic is based at 430 Fisher Creek Road in the offices of Sylva Family Practice in Sylva. Sylva Family Practice has been tremendously supportive of Palliative Medicine and our practice in particular.
To be seen in clinic: We will need a referral from your primary care or sub-specialist provider stating reason for consultation (usually symptom management and/or assisting with goals of care); a “Face Sheet” that has your address, insurance information, etc; and the providers last note regarding your care. These can be faxed to: 828 586-5162.
